The aim of the annual education review in Year 9 and subsequent years is to review the young person's statement and to draw up and subsequently review the Transition Plan. This should involve the agencies that may play a major role in the young person's life during the post-school years and is co-ordinated by Connexions and in partnership with the school.
Assessment of need by Social Worker
To avoid unnecessary duplication this assessment should be completed in tandem with the LAC Review, the Educational Review and the Personal Education Plan for the young person. Clearly the Children and Young People Service (C&YPS) cannot commit the resources of other agencies. All that can be recorded is that the social worker will act as advocate to try and ensure those resources are provided and/or what has been agreed by other agencies at that time.

If the young person is Disabled and has a level of need that would require ongoing personal care, the relevant adult care team should be notified by the young person's 16th birthday and a full Community Care Assessment ought to be undertaken under the NHS and Community Care Act 1990 before they reach 17 years 6 months. If on the basis of the assessment it is judged that their continuing personal care needs can be met by their former foster carers, arrangements should be made by the relevant Adult Care Team for the necessary registration requirements to be completed and payments to be made in accordance with Adult Care payment levels from the relevant Adult Care Budget. These arrangements to be subject to review in the normal way by Adult Services.
If the Young Person was in care on their sixteenth birthday then they may be eligible for a service by the Care and After Team within the C&YPS - the young person's social worker would be responsible for making a referral to this team,
The young person's social worker should initiate all referral procedures if it is not triggered by actions of the young person's school or Educational Psychologist from the CYPS in the Review of Statement of Special Educational Needs Procedure.
