For a child looked after, a health assessment should be in place in time for a written report and health plan to be available for discussion at the child’s first review. The progress made in implementing the plan should be considered at the second and subsequent reviews and the Health Plan modified in the light of any additional information collected.
Children Looked After Nurses (CLAN) give advice and support to looked after children, carers, social workers and other professionals regarding health issues as well as administrating the Health Assessment Process.
