8. After Care

All young people who have been looked after by Bristol for more than 13 weeks between their 14th and 16th birthdays (excluding short respite care placements) and who are still looked after on their 16th birthday will receive support from the Care & After Team from age 16-21 (or up to 24 for young people in full-time education, or 25 if young person has a disability).

The Pathway Plan (y) – Part One is the young person's needs assessment which is completed by the social worker. Part Two is completed by the Care and After Team worker or Personal Adviser.

My Pathway Plan is a consultation document for these young people.

The Pathway Plan Review (Ya) is the review of this plan.
