- Social worker should phone Kate O'Brien, Adoption Adviser, or Brenda Monksfield, Adoption Administrator, on 0117 903 7782/4 to book Adoption Panel.
- Social worker books Adoption medical.
- Social worker completes Child's Permanency Plan.
- Team manager approves Child's Permanency Plan.
- Social worker sets up Adoption file (red binder) for linking information on paper, secured in a separately locked place.
- Social worker to arrange (with admin) that child's PARIS record is carefully checked for potential breaches of confidentiality arising from information linking prospective adopters
- Adoption Panel considers Child's Permanency Plan, makes recommendation that “child be placed for adoption” and sends report to Agency Decision-Maker to approve recommendation.
- Agency Decision-Maker approves recommendation.
- Adoption senior practitioner sets up Matching meeting, chairs meeting, then writes Adoption Placement Report that recommends linking child to prospective adopter(s).
- Adoption Panel considers Adoption Placement Report, endorses recommendation and sends report to Agency Decision-Maker to approve recommendation.
- Agency Decision-Maker approves recommendation.
- Adoption senior practitioner sets up and chairs introductory planning meeting with prospective adopters and writes Adoption Placement Plan.
- Adoption Placement Plan approved by social worker's team manager.
- Social worker places child with prospective adopters.
- Placement Information Record (O) form to be printed off and filled in on paper copy to go to Adoption file, with copy for prospective adopters.
- All professionals to continue to apply children looked after procedures. Social worker continues statutory visits to the child. Independent Reviewing manager reviews adoptive placement after 28 days, three months and six monthly thereafter until Adoption Order is obtained by prospective adopters.
