Social Services request the support of colleagues in getting the names of disabled children onto the Register. It is in our interest to make sure that the register is comprehensive and up-to-date, so that it can be used for planning appropriate provision.
The attached leaflet should be brought to the attention of parents of disabled children and staff who work with families where there is a disabled child. The leaflet includes Registration Form 36 and should be self-explanatory but if you have any queries please contact the Social Services Disabled Children's Register Clerk on tel: (0117) 903 8250. She will be able to advise you about how to proceed.
The Bristol Register of Disabled Children has a simple registration procedure which is based on the parents' assessment of their child's impairment. If someone is in doubt about whether or not their child's name is on the Register please advise them to send in a completed Form 36 anyway so that Social Services can update their records. A registration card is sent to families on receipt of completed application form.
